You are a carer if you look after someone who is reliant on your help because of their pre-existing illness, frailty or disability. The care you provide is unpaid.
A carer's knowledge and experience can be enormously beneficial to the patient if they are admitted to hospital as they can often help inform nursing and other staff members about the most appropriate treatment and care plan for the patient’s best interests.

By adopting the passport, carers will be able to agree their level of involvement in the patient's care plan and will feel fully informed about what additional help is available to them.
