Subject Access Request - Personal Data

Subject to certain exemptions, you have the right under General Data Protection Regulation (GDPR) to be told whether East Cheshire NHS Trust holds any information about you (your personal data) and a right to be provided with a copy of the information within one month (30 days). Your subject access application will be free of charge unless it is manifestly unfounded or excessive and we will require sight of an official document such as a passport or drivers licence to verify your identity.

To assist us with locating the information required, please provide as much information including dates (where possible). For example: Personnel file from 2005 to 2008. Date left the Trust 12/12/2008. Department title and name of manager.

Please provide copies of two official documents which between them provide sufficient information to confirm your name, date of birth, address and signature.
