Discharging a patient

Your estimated discharge date should be written behind your bed.  This will provide you with an idea of how long we expect you to remain in hospital.

Each morning we have a multidisciplinary team meeting called a ‘board round’, this takes place in a separate room to maintain confidentiality and is chaired by the senior sister or her deputy.  At this meeting we discuss each patients’ current medical condition and the plan for the day and any expected discharges.  At this meeting we also decide which patients may need assessment from our therapy team (Physio or OT), which patients may need additional help at home, additional rehab or 24 hour care.  Our aim will always be the least restrictive option for each patient and any decisions which need to be made will be discussed with the patient and their family.

On the day of your discharge you may need to be seen by the doctor to make sure they are happy you are well enough to go home.

The doctor will write your discharge letter and prescription.

The ward pharmacist will check your prescription and prepare your medications to take home.

Once your medications are ready you will be able to go home and you will receive a copy of the discharge letter which also gets sent electronically to your GP.

You will be asked to move to the discharge lounge located downstairs whilst you are waiting for your medications and/or transport.

We request patients arrange their own transport home and for family members to make sure you have the appropriate outdoor clothing brought in ready for your discharge.

Additional information

We do not have laundry facilities on the ward or in the hospital, please keep laundry in a separate bag and if anyone can take this home for washing it is appreciated due to the lack of storage space on the ward.

At the entrance to the ward there is a board on the left-hand side which contains photographs of all the staff which you might find helpful.

Although there are 24 beds on the respiratory ward, sometimes we do have additional beds open if the hospital has more medical patients than we have beds for.  This can mean that ward 11 may have one or two extra bays of 6 patients, the care and management of these patients will remain the same, but they would have a different Consultant to those named above, most likely a medical Consultant rather than respiratory.

If you, or a family member, wishes to speak to a member of staff about your nursing care please speak to the nurse looking after you.  Alternatively, you can request to speak to the nurse in charge that day.  Or, if you would like to speak to someone more senior then please ask to speak to one of the Ward Sisters or the Senior Sister.  It is important we follow this escalation process as this is the trust standard for all the wards.  If you require more senior support beyond the above, we will then help you to escalate your concerns to the Matron within working hours or site manager out of hours.

If you, or a family member, wishes to speak to a member of staff about your medical care please speak to the nurse looking after you and they will ask one of the doctors to help answer your questions.
